Instructions for using Consent Templates
General
- Do not leave any sections blank. Use complete sentences.
- Use 12 point font. Times New Roman or Arial font is recommended.
- Use simple lay language and spell out all abbreviations. For consent and parental permission templates, use an 8th grade reading level throughout.
Line Numbers
- Line numbers have been added to aid in the review process and must be removed after the consent has been approved by the IRB.
- To remove line numbers from the template:
Using Microsoft Word 97-2003: From File > click Page Setup > “Layout” Tab > “Line Numbers” Button > Uncheck “Add Line Numbering” > Click “OK” to close dialog boxes
Using Microsoft Word 2007: From Page Layout > select “Line Numbers” > select “None”
Template Header
- To add information to the template header:
Using Microsoft Word 97-2003: From View > Header and Footer > add information to appropriate header fields and click “Close.”
Using Microsoft Word 2007: From Insert > select “Header” > select “Edit header” > add information to appropriate header fields and click “Close.”
- IRB Protocol Number – the number assigned by ORRP (e.g., 2008H0156)
- IRB Approval date – the date of IRB approval, which can be found on your approval letter
- Version – personal tracking mechanism; dates (e.g., 11/20/2009) or numbers (e.g., 1.2)
