Updates to Exempt Application
October 20, 2009
The OSU Exempt Application has been updated to support various process changes within the University. A summary of the major changes is provided below:
- Deleted instructions and exempt categories pages (content can be found online)
- Added link for "Qualifications for Service as a PI"
- Added a section to collect the names of key personnel. Also added a prompt to complete Appendix A1 for additional co-investigators or key personnel (more than 1 of each)
- Added definition of key personnel
- Added financial conflict of interest question, note, and examples to reflect process change
- Updated screening questions to remove pregnant women as an exclusion
- Revised terms "subjects under the age of 18" to "children" and "subjects over the age of 18" to "adults"
- Added prompts to complete Appendix N if the research involves PHI and Appendix U for international research
- Revised general question #1 to require attaching a copy of the research proposal
- Other minor changes made, including reformatting, addition of clarification statements/terms, and updated hyperlinks
Effective Date: November 1, 2009 (new version must be used by this date)
