IACUC Personnel Addition / Removal Form

An IACUC Personnel Addition/Removal Form should be utilized for adding or removing personnel, not including the Principal Investigator, from an Animal Use Protocol. A change in the Principal Investigator of an Animal Use Protocol must be processed utilizing the IACUC Amendment Form.
