IACUC Housing Exception Request Form

The housing of animals outside of an approved OSU Farm Facility or an ULAR animal facility for greater than 12 consecutive hours must be approved by the IACUC. Submission of the Housing Exception Request Form and a set of standard operating procedures for animal husbandry (example available here) are required.
