Tips and Directions for Downloading Forms
Form Formats
All downloadable forms on this site are saved in either 
PDF or 
Microsoft
Word format.
Adobe PDF Format
In
order to view, print, search, and fill-in PDF forms you will need a copy
of Adobe© Acrobat© or the Adobe© Reader©.
If you do not have the latest version of the Adobe© Reader©,
click the icon to go to the Adobe© web site where the latest version of
the Reader can be downloaded for free.
Microsoft Word Format
In order to view, print, and fill-in Word formatted forms you will need a copy of Microsoft Word. If you do not have Microsoft Word you can download the Word
Viewer 2003 for free.
Downloading Forms
- Right-click on the link and select "Save Target as..." or "Save Link Target as..."
- A dialog box will appear and you will be prompted to save the file to your computer. Choose a location to save the file.
- Open the file from your computer using Microsoft Word or another compatible word processing program and complete the application form.
Troubleshooting
Computer security systems such as firewalls can prevent you from accessing the forms. If you are not able to open or download a document, please contact the RF Help Desk (688-8288 or rfhelpdesk@osu.edu) or send an e-mail message to rrpwebmaster@rf.ohio-state.edu.
Additional Requirements
Due to signature requirements, the Office of Responsible Research Practices cannot accept these forms as e-mail attachments.
