IACUC Amendment Request Form
The IACUC Amendment Request Form must be submitted to notify the IACUC of any proposed changes regarding the work described within an IACUC Animal Use Protocol. It is a direct violation of OSU, IACUC, and Federal Policies to implement a change to an Animal Use Protocol until such change is approved by the IACUC (except where absolutely necessary to eliminate apparent immediate hazards to individual(s) and/or animal(s)).
Beginning January 1, 2009, a ULAR clinical veterinarian signature is required prior to submission to the IACUC Office if sections 6-10 of the IACUC Amendment Request Form are completed.
